Some fractures are simple breaks that heal well in a cast. Complex fractures are different: the bone may be broken into many pieces, the break may run into a joint, or the skin and soft tissues may be badly injured.

These fractures need careful planning and precise surgery to rebuild the bone and the joint surface, so that the limb heals straight and the joint moves well afterwards.

Seek urgent care: for any fracture with a wound over it (an open fracture), severe swelling with increasing pain, numbness, or a cold or pale limb. These need emergency treatment.

What Makes a Fracture Complex?

  • Periarticular and intra-articular fractures: breaks around or into a joint, such as the knee, elbow, ankle, wrist or shoulder
  • Pelvic and acetabular fractures (the hip socket)
  • Comminuted fractures, where the bone is broken into several pieces
  • Open fractures, where the bone has broken through the skin
  • Fractures with a dislocation of the joint
  • Fractures in weak, osteoporotic bone in older patients
  • Several fractures at once after a high-energy accident

Why Joint Fractures Need Precise Surgery

When a fracture runs into a joint, the smooth cartilage surface is broken. If the pieces heal even slightly out of place, the joint can become stiff and painful and develop arthritis early.

Surgery aims to put the joint surface back together accurately and hold it firmly enough for movement to start early. Early movement is what prevents stiffness.

How Complex Fractures Are Treated

Every complex fracture is planned individually using X-rays and a CT scan.

Staged surgery

When swelling or skin damage is severe, a temporary external frame first holds the bone out to length. The final fixation is done once the soft tissues are safe, usually within 1–2 weeks.

Plate and screw fixation

Pieces of the joint surface are put back in place and held with plates and screws. Sometimes two plates are used (double plating) for extra stability.

Intramedullary nailing

A rod inside the bone for fractures of the shaft of long bones such as the thigh and shin bones. It allows early walking in many cases.

Minimally invasive fixation

Where suitable, plates are slid in through smaller cuts to protect the blood supply to the bone.

Bone grafting

Bone may be added to fill gaps where the bone has been crushed, to support healing.

Open fracture care

Urgent antibiotics, thorough cleaning of the wound in theatre, and stabilisation of the bone to lower the risk of infection.

What is a periarticular fracture?

A periarticular fracture is a break close to a joint, and an intra-articular fracture runs into the joint surface. Both need precise fixation to keep the joint moving well and to reduce the risk of early arthritis.

Why is a CT scan needed before fracture surgery?

A CT scan shows the exact number, size and position of the fracture pieces, especially in joint and pelvic fractures. It allows the surgery to be planned in detail.

Why is my surgery being done in two stages?

When the skin and soft tissues are badly swollen or injured, operating straight away increases the risk of wound problems. A temporary frame holds the bone first, and the final fixation is done once the tissues have settled.

Will the plates and screws need to be removed?

Most implants stay in place for life without problems. They are removed only if they cause discomfort or other issues, usually after the bone has fully healed.

How long does a complex fracture take to heal?

Many fractures heal in about 3 months, but complex and joint fractures can take longer. Recovery of full strength and movement often continues for several months with physiotherapy.
